Nexus Letters & DBQ Services for Veterans

At ANM Health LLC, we understand how important accurate documentation is for your VA disability claim. Our goal is to ensure that your experiences are heard, your symptoms are validated, and your evidence is presented clearly and professionally.

 

Our Nexus Letter and Disability Benefits Questionnaire (DBQ) services are designed to support your case with the clinical clarity and objective documentation the VA looks for.

What is a Nexus Letter?

A nexus letter is a professional medical opinion that links your current condition to your military service. It explains—using clinical evidence—how your symptoms are at least as likely as not connected to your service history.

 

We use a structured, evidence-based approach to ensure accuracy and credibility in every letter.

What is a DBQ?

A Disability Benefits Questionnaire (DBQ) is a standardized VA form used to document your symptoms, diagnoses, and level of impairment.

When completed by a licensed clinician, a DBQ provides the VA with clear, objective data to support your disability rating.

 

DBQs can be completed alone or in combination with a nexus letter to strengthen your claim.

Our Process

 

  1. Intake & Record Review: Outreach our Veteran Liaison, Marc to begin the intake process.  We will provide you with requirements and schedule your appointments.  Following the intake process, we review your service records, prior evaluations, and VA decision letters.

  2. Clinical Evaluation: A comprehensive visit with Ashleigh to document your condition and service connection. (billable to insurance if applicable)

  3. Letter & DBQ Preparation: Ashleigh completes the medical opinion and supporting documentation using VA standards.

  4. Review & Delivery: You receive a signed, finalized document ready for submission to the VA or your representative.

​Transparent Pricing

Initial Clinical Evaluation: $300 cash pay or billable to insurance if applicable

Comprehensive review to establish diagnosis and service connection

Nexus Letter: $750 cash pay; billed at time of scheduling

1–2 conditions; detailed record review, diagnostic summary, and clinical rationale

Add-On Condition: +$200 each; cash pay; billed at time of scheduling

Additional diagnosis or condition addressed in the same letter.

DBQ Completion (alone): $400 cash pay; billed at time of scheduling

Completed from your clinical evaluation or provided documentation.

Nexus Letter + DBQ Combo: $1000 cash pay; billed at time of scheduling

Combined service with streamlined evaluation and documentation.

Rush Service (7-day turnaround): +$150 cash pay; billed at time of scheduling

All documentation services are non-billable to insurance and payment is required at the time of booking.

​

Timeline

Standard turnaround time is 2–3 weeks from the date of your evaluation and receipt of all required records.

Rush service may be available depending on case complexity and provider capacity.

All documentation services, including nexus letters, DBQs, and record reviews, are non-refundable once the evaluation and/or preparation process has begun. This policy ensures fairness and transparency, as the time and professional expertise required to review records, assess conditions, and prepare documentation are significant and cannot be reversed once services are rendered.
